IF YOU HAVE REASON TO BELIEVE THAT YOUR LICENSE HAS BEEN SUSPENDED BY THE DEPARTMENT OF PUBLIC SAFETY AS A RESULT OF A TRAFFIC CONVICTION AND/OR FAILURE-TO-APPEAR, YOU WILL NEED TO PROVIDE THE COURT WITH SOME AND POSSIBLY ALL OF THE FOLLOWING INFORMATION:
